This document was created by a committee of law enforcement from all jurisdictions of Jefferson County, Colorado, victim advocates, the District Attorney's Office and pretrial services. This four-page Domestic Violence Case Investigation Form is a resource for officers who assess and document domestic violence assaults. This template resource contains sections for officers to document the scene, conduct a risk assessment, log victim and suspect information, and identify prior domestic violence behaviors and incidents. This document also includes a diagram for victims to identify and log their own injuries, as well as a page for officers to collect a victim and/or witness statement.
